Use these Google forms to enter maintenance request progress:
- Use FLCC Maintenance Request (Paper) to initially enter the information from the top half of a paper ticket.
- Use FLCC Maintenance Request Progress to add information to an existing maintenance request (whether from the bottom half of a paper or printed ticket).
After submitting a Google form, refresh this page to see your additions in the list below, Note: Google rebuilds the list immediately after the first form submission, but then only every 5 minutes thereafter.
